Waiver Form – Plan Policy

Terms and Conditions

1. Assumption of Risk: I, as the parent or legal guardian of the participating child, acknowledge and understand that participation in the Soccer Heroes LLC sports programs involves inherent risks. These risks include, but are not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and death. I understand that these injuries or outcomes may arise from my own or others' negligence, conditions related to travel to and from the activity, or from conditions at the activity location(s). Nonetheless, I assume all related risks, both known and unknown to me, of my child participation in this activity. I voluntarily allow my child to participate in the program, understanding and accepting these risks.

 

2. Medical Fitness: I certify that my child is physically fit to participate in the Soccer Heroes LLC sports program. I understand that it is my responsibility to inform the program staff of any pre-existing medical conditions, allergies, or injuries that may affect my child's participation.

 

3. Release of Liability: In consideration of my child's participation in the Soccer Heroes LLC sports program, I hereby release, discharge, and hold harmless Soccer Heroes LLC, its instructors, coaches, staff members, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of any injury or harm that may occur during or as a result of my child's participation in the program.

4. Parental Consent: I give my consent for Soccer Heroes LLC to provide necessary medical treatment or emergency care for my child in the event of an injury or medical condition requiring immediate attention. I understand that Soccer Heroes LLC will make reasonable efforts to contact me in such situations.

5. Code of Conduct: I understand that my child is expected to follow the code of conduct established by Soccer Heroes LLC. This includes displaying good sportsmanship, respect for coaches and fellow participants, and following the rules and regulations of the program.

6. Photography and Media Release: I grant Soccer Heroes LLC the right to use photographs, videos, or other media featuring my child for promotional purposes, including but not limited to website, social media, and marketing materials.

7. Program Changes and Cancellations: Soccer Heroes LLC reserves the right to modify, reschedule, or cancel the sports programs sessions or events due to unforeseen circumstances, including inclement weather or insufficient enrollment. In such cases, Soccer Heroes LLC will make reasonable efforts to provide alternative options or refunds.

I have carefully read and understood the above waiver and terms, and I voluntarily agree to its provisions. I acknowledge that by signing up my child for the Soccer Heroes LLC sports program, I am legally bound by this agreement.
